You could remove the local fh entirely, if you change

	return v4l2_m2m_streamon(file, fh->m2m_ctx, type);

to

	return v4l2_m2m_streamon(file, channel->fh.m2m_ctx, type);

in the allegro_ioctl_streamon() function.
